Certified copies of Academic Certificates and Transcripts attested by Somali ministries of Higher Education
Curriculum Vitae (CV)
Internship Certificate/Training Period Certificate
Secondary school certificate
Copy of Identity card or passport.
2 Recent Passport size photographs.
Criminal Clearance Letter
If studied outside the country attestation from the Embassy

Registration requirements for health Facilities
Valid Local Government Registration Certificate (one that has not expired).)
Completion of the Application Form for the Registration of Health Service Centers.
Ownership Title Documents for the health center.
A Complete Profile/Document describing the health service center’s activities (services provided) and its number of employees.
Copy of Previous Registration, if the center was previously registered with the National Health Professionals Council.
Criminal Record Clearance (Certificate of Good Conduct) for the Owner.
